If you would like to become a member of St. Patrick Parish, please print the Parish Registration Form.  Place the completed form in the collection basket.

If you are currently a parishioner and any of your (or your family's) information on file with us has changed, please complete the form, so that we can update our records. If you have added a family member, be sure and let us know his/her full name, gender and date of birth.
